Support at Home

Support at Home is an Australian Government program designed to help older people remain living safely, independently and comfortably in their own homes for longer.


At Bilingual Care, we provide personalised support based on your individual needs, goals and preferences. Our team can assist with everyday activities, personal care, domestic support and other services that help you maintain your independence and quality of life.


Our Support at Home services include:

  • Personal care and assistance with daily activities
  • Domestic Assistance, cleaning and laundry
  • Shopping and meal preparation
  • Transport and assistance to attend appointments or community activities
  • Home maintenance and gardening
  • Nursing and other clinical supports
  • Allied health services, including physiotherapy, occupational therapy, podiatry and nutrition support
  • Social support and assistance to stay connected with your community
  • Care management and coordination


The services available to you depend on your assessment, support plan and individual care needs. Support at Home can also provide access to equipment, assistive technology and home modifications where approved.


How does it work?

To access Support at Home, you first need to be assessed through My Aged Care. If you are approved, you can choose a registered provider to deliver the services included in your support plan.

Funding levels & categories

Your funding, your choices

If you are approved for ongoing Support at Home, you will receive one of 8 funding classifications based on your assessed level of need. Each classification has a quarterly budget that can be used for approved services in your support plan.  You work with your chosen provider to decide how your approved services are delivered and how they fit into your goals and daily life.

Categories:

If you are eligible for government funding under Support at Home, your funding can be used to access a range of services, including nursing care, assistance with daily activities and support to maintain your independence at home.


Services are divided into three core categories:

  • Clinical Services

Professional health and allied health services designed to support your health, wellbeing and functional abilities. This may include nursing, physiotherapy, occupational therapy, podiatry, nutrition and other clinical supports.


  • Independence Support

Practical support that helps you maintain your independence and continue doing the things that matter to you. This may include personal care, transport, social support, respite and assistance with daily activities.


  • Everyday Living

Support with everyday tasks to help keep your home safe, comfortable and enjoyable. This may include domestic assistance, meal preparation, shopping, laundry, gardening and home maintenance.

Funding levels:

Eligible participants are placed into one of eight funding levels. The amount of funding available depends on the level of support required.

  • Level 1 - $11,010.01
  • Level 2 - $16,451.35
  • Level 3 - $22,536.81
  • Level 4 - $30,468.51
  • Level 5 - $40,729.53
  • Level 6 - $49,365.27
  • Level 7 - $59,660.00
  • Level 8 - $80,137.12

How to Access Support at Home

Start by checking whether you are eligible for an aged care assessment. You can do this through My Aged Care online (https://www.myagedcare.gov.au/how-get-assessed) or by calling 1800 200 422.

You can apply online (https://www.myagedcare.gov.au/apply-online), over the phone (1800 200 422) or in person. When applying online, you will generally need your Medicare card and some basic information about yourself and your current situation.

An assessment organisation will contact you to discuss your needs and arrange your assessment. Assessments are usually completed in your home, where an assessor will talk with you about your health, daily activities, safety, current supports, goals and preferences.


You can have a family member, friend, carer, supporter or interpreter with you if you need assistance.

Following your assessment, you will receive an outcome that explains the services you have been approved for and your individual support plan. The plan is based on your strengths, needs, goals and preferences.

Once you are approved and your funding is available, you can choose a registered Support at Home provider that suits your needs and preferences. You can consider factors such as the services offered, location, language and cultural understanding, and the level of support provided.

Thinking About Switching Providers?

Your care should work for you. If your needs, preferences or circumstances change, you have the right to choose a different Support at Home provider at any time. You cannot be charged a fee for leaving your current provider.


Before making the change, it is recommended that you find your new provider first and discuss an end date with your current provider. This helps minimise any interruption to your services.
